If you have any questions, please contact us at 224-253-8805. Follow these steps to sign up for a MyChart account. Enter your personal information. Verify your contact information. Choose a username and password. Connect to Your Provider Sign up for MyChart or log in with a healthcare provider below. Where do you receive care? MyChart is a service your healthcare organization provides to give you access to your health record. Your records stay at the organization where you receive care. Some MyChart features may not be available at all healthcare providers. Get answers to your medical questions from the comfort of your own home No more waiting for a phone call or letter – view your results and your doctor’s comments within days. MyChart lets you see your medications, test results, upcoming appointments, medical bills, price estimates, and more all in one place, even if you’ve been seen at multiple healthcare organizations. Your healthcare provider has access to your full medical record so they can provide the best care for you. If you’re seen at multiple organizations, your provider might be able to see information from your record at another organization if both organizations participate in Care Everywhere. Enter your MyChart username and password, and click Sign In. Download the MyChart mobile app from the Google Play Store (for Android) or the App Store (for Apple iOS). Open the app and search for your healthcare organization. You can search by location or by organization name. MyChart is powered by Epic, the software system used by the world’s best healthcare organizations.
